How It Works

The Admissions & Intake Process

Our admissions process is designed to be clear, collaborative, and pressure-free. We work alongside families and DDA coordinators at every step to make sure the placement is right for everyone involved.

STEP 1

Referral Submission

Family or DDA Coordinator (CCS) submits clinical records and Person-Centered Plan (PCP). We accept referrals by phone, email, fax, or through our online inquiry form.

STEP 2

Intake Assessment

Our clinical team conducts a thorough assessment to verify home and care compatibility — reviewing medical history, behavioral needs, and personal preferences.

STEP 3

Plan Alignment

We collaborate with guardians, case managers, and the resident to align on daily care routines, personal goals, and service expectations before any commitment.

STEP 4

Transition & Move-In

We coordinate a smooth, comfortable transition into our community — including a home orientation, staff introductions, and a personalized first-week schedule.

Our Residence

A Home, Not an Institution

Our Baltimore County residence is designed to feel like a real home — because it is one.

Comfortable, Accessible, and Welcoming

Every detail of our residence was chosen with residents in mind — from the layout of common areas to the warmth of a home-cooked meal.

  • Private and semi-private rooms with personal furnishing options
  • Spacious common areas for socializing, activities, and family visits
  • Fully accessible facilities throughout the residence
  • Chef-inspired, dietitian-approved meals prepared daily
  • A calm, family-like atmosphere — not an institutional setting
Common Questions

Frequently Asked Questions

Answers to the questions families and coordinators ask most often during the placement process.

To begin the referral process, we typically need the individual's Person-Centered Plan (PCP), recent medical records, a behavioral assessment (if applicable), current medication list, and the DDA waiver enrollment confirmation. Your CCS coordinator can help compile these documents. If you're missing something, reach out anyway — we'll work with you to gather what's needed.

The timeline varies depending on the completeness of the referral packet and bed availability, but most placements move from initial referral to move-in within 2 to 4 weeks. We prioritize thorough assessments over speed — the goal is to ensure a good fit, not to rush the process.

Absolutely. We encourage families and guardians to tour the residence, meet our staff, and spend time in the common areas before committing to a placement. We want you to feel confident and comfortable with the environment your loved one will be living in.

Person-Centered Plans are living documents. We conduct regular reviews with families and DDA coordinators to reassess goals, adjust care strategies, and ensure our services continue to align with each resident's evolving needs. If a resident's support level changes, we adapt — that's what person-centered care means.

Residential services at ZadkeilCare are funded through Maryland's DDA waiver programs (Community Pathways and Family Supports). Eligible individuals receive services at no direct cost to families. Your CCS coordinator can verify enrollment and funding eligibility as part of the referral process.
